Renal Replacement Therapy (RRT) is a life saving treatment for patients with endstage renal disease (ESRD). But both forms of RRT, Repeated Dialysis Treatment (RDT) or Peritoneal Dialysis (PD) and Organ Transplantation (Donor Kidney: Cadaver Donor CD, Living donor LD) do not come without severe sideperformance. Further technological improvements (bio-compatibility, portable kidney, specific immunological suppression) are needed but will not totally eliminate the side-effects of RRT. There is a high demand for clinical-ethical leadership towards patients and team. Clinical-ethical aspects have to be integrated into future nephrological research. I propose clinical-ethical Action Guides for a patient centered dialysis treatment, 5 rules for the physician and 5 for the patient.
