Quality standards and norms developed in evidence based therapy have improved to safety and validity of medical judgment and intervention. However, in patient-oriented medical treatment general quality standards have to be applied according to the special situation of the individual patient. Therefore, ethically required and practically useful general rules of conduct need to be implemented by rules of implementation in yet to be developed concepts of clinical indications recognizing the special situation of the individual patient.
