Value Anamnesis severs as an instrument for the patient to recognize her or his personal system of values and criteria for quality of life and to introduce those into the patient-physician interaction. The Evaluation Form confronts the patient in a non-acute situation with narrations of complex situations of terminal care and calls for her or his personal reaction. This Evaluation Form does not provide for ready-made treatment instructions, it requires a careful diagnosis in terms of differential ethics replacing traditional forms of inflexible Advance Directives.
